Plan brochures you can download
These are the carrier brochures for two of the alternative health plans Jennifer offers, reproduced exactly as the companies published them. Her name and number are on the cover of each one, so read them, then call her with your questions rather than the carrier.

New Era Optimum Health Saver
A hospital indemnity plan from Philadelphia American Life. It pays a set dollar amount for each covered service instead of a percentage, so the figure is known in advance. Compare the three benefit levels in the tables, then read the worked examples near the back, which show what an office visit, a broken arm and a seven-day hospital stay actually pay out.
The brochure states this is an excepted benefit plan that does not meet the requirements of the Affordable Care Act. It is not a substitute for major medical cover. Ask Jennifer how it fits alongside what you already have.

SmartHealth healthcare benefit plans
Four levels of benefit plan, from virtual care only up to comprehensive, plus dental, vision and accident add-ons. Start at the comparison grid a few pages in, which shows at a glance which benefits each level includes and what each one costs a month, then read the summary pages for whichever level fits.
The brochure states these are not major medical insurance and are not ACA-qualified plans, and that the comprehensive part of the top plan is a medical share plan rather than an insurance policy. Worth asking Jennifer about before you enrol.

SmartHealth monthly rates
The price list for every SmartHealth plan above. Find your plan, then your age band and whether you are covering yourself, a spouse, children or the whole family. One page.
These rates are marked effective 21 June 2025, and the sheet itself says the rate at the time you enrol replaces the one shown. Treat the figures as a guide and confirm the current price with Jennifer.
